- Conducts pre admission planning
- Conducts psycho-social assessment, level of psycho-social functioning and financial eligibility according to AO 51-A s.2001
- Mobilizes resources to meet medical needs of patients and their families.
- Prepares required recordings and reports.
- Coordinates and collaborates with concerned multi disciplinary team members to provide holistic care to patients and their families.
- Provides appropriate concrete services.
- Provides SW intervention to disadvantaged and abandoned patients.
- Provides information/education to patients and their families.
- Compliance to IMS (Integrated Management System) Standards and Policies.
- Performs other related functions that may be assigned from time to time
